College Student Volunteer Program Information:

NOTE: Use this application only if you are a full-time, local college undergrad (Sophomore, Junior or Senior) .

NOTE: All evening and weekend volunteer opportunities are currently full.
Thank you for your interest in Cook Children's college student volunteer program. Cook Children's is a non-profit hospital that has been caring for sick and injured children since 1918. From the very beginning, volunteers have played a vital role in providing comfort and care during a time of need.

Volunteer opportunities are currently available for local college students Monday - Friday, mornings and afternoons. Shifts are usually once per week at the same day/time, although some opportunities exist with more flexibility.

College Student Program Requirements:
1. Must be 18 years old and out of high school,
2. A current full-time (12 hours or more) undergrad Sophomore, Junior, or Senior enrolled in college courses.
3. Previous experience with children preferred.
4. Able to commit to volunteer a minimum of 50 hours from September 1, 2019 to May 30, 2020. The program allows for the student to have the summer off.

College Student Program Health Requirements:
1. A copy of your immunization record including varicella (chicken pox) immunity must be provided to Cook Children's. This can be one of the following:
A. Proof of having received the two varicella vaccinations from your medical/immunization record.
B. Proof of a positive blood titer (blood work) showing that you are immune to varicella from your doctor, a lab, or the Health Department. If your blood work shows you are not immune (negative), Cook Children's will provide the varicella vaccinations to you at no charge.
We can provide you with a location near Cook Children's that will do a blood titer at a reasonable cost ($40), if needed.
2. Cook Children's requires volunteers to complete an annual Tuberculosis (TB) skin test and flu vaccination. These are provided at no charge.

College Student Onboarding Process:
1. Applications will be reviewed after the July 1st application deadline.
2. When the application and references have been reviewed for a possible match, the student will be contacted by e-mail for a personal interview.
3. At the time of the interview, the student will be asked to sign an Authorization for Release of Information so that a background check can be completed.
4. If the student is accepted into the program, the student will be invited to attend orientation(s).
5. Those who are not accepted will be notified by e-mail.

Background Check

You will be asked to sign an authorization form (at your interview) allowing your background check to be completed. Applicants must have a clear record (no arrests, convictions, nolo contendere pleas, deferred adjudications, or any other criminal entries). Per Cook Children's policy, you will be declined as a volunteer if there are any blemishes on your background check.

I understand that false or misleading statements or the omission of any information necessary to make this application complete will result in rejection of my application or termination of my service.

Additionally, Cook Children's has my consent to photograph, videotape, or audiotape me performing my volunteer duties. I understand that these may be used toward the advancement of public education, the promotion of Cook Children's, and/or any other legitimate purpose.

I understand that upon my successful completion of the volunteer placement processes required at Cook Children's, I will become a volunteer. As a volunteer, I acknowledge that I will not receive compensation for services.
